Job Description:

The Patient Educator (RN) provides individualized patient nursing education to patients and families with diabetes mellitus in classroom and individual settings in accordance with American Diabetic Association (ADA) and Diabetes Self-Management (DSME) guidelines, AADE and Sweet Success Gestational Diabetes as applicable. Educates patients and families on initiation of insulin therapy insulin pump and continuous glucose monitoring systems. Assists with education of clinical professionals. Provides nursing care in accordance with professional, regulatory standards and evidenced based practice. Maintains the standards of nursing care as recommended by the Nursing Department’s philosophy and objectives. Ability to function in in-patient and out-patient environments. Involved in the planning and development of telehealth capacities at the Center to potentially conduct consultations via telephone and video to patients. Demonstrates the ability to independently provide health assessment, RN treatment in the ambulatory and/or in-patient care setting and through the use of telehealth.


Essential Functions:


Clinical & Teaching

• Uses best practice methods for assessment, plan and reassessment of patient needs utilizing the nursing process in association with DSME and/or Sweet Success recommendations to achieve desired goals for patients with diabetes mellitus and manifestations.

• Includes within the treatment plan the assessment of social, economic and religious needs.

• Develops education programs with written descriptions and learning tools of purpose to participants.

• The program content reflects relevant input from participants and is interactive.

• Organizes learning experiences to help participants understand the relationship of concepts and application to real-life situations and bases content on current data, relevant to learner’s needs.

• Provides measureable learning objectives for the entire program to assess the efficacy of the tutorial.

• Able to convey knowledge to learner’s in a non-judgmental manner that encourages questions.

• Medical record documentation is completed on the same day of patient visit to optimize continuum of care according to nursing procedures and DSME and/or Sweet Success standards.

• As Diabetes Educator, serves as liaison among program staff and staff of clinical services, physicians, Education Department and community. Works collaboratively with all groups to meet patient needs and achieve positive patient outcomes.

• Monitors and evaluates the effectiveness of the case management plans and modifies as necessary.

• Acts as a patient advocate and educator to assure that the patient has the knowledge to care for his/her condition and patient is educated and empowered to be responsible for participating in the plan of care. Actively reaches out to discharged patients, home health entities, and/or nursing facilities as part of the coordination of care.

• Develops individualized patient/family education plan focused on self-management, delivers patient/family education specific to a disease state.

• Develops and updates training and educational materials and presents to appropriate staff, members and families. Facilitates patients' return to normal daily activities by teaching and making appropriate referrals for outside services/continued care.

• Consults with internal and external physicians, health care providers, discharge planners, and outside agencies regarding continued care/treatment or hospitalization or referral to support services or placement.

• Participates in data collection and analysis of clinical outcomes of care and customer satisfaction standards. Participates in the formulation and implementation/monitoring of action strategies and outcomes of care or customer service. Ensures that accurate records are maintained of the care associated with each patient.


Team Interactions, Community Relations & Leadership

• Collaborates with other disciplines within the Hoag inpatient setting to facilitate to ensure continuity of care throughout the in-patient and out-patient experience.

• Functions as internal consultant and change agent in organizational activities by functioning as a mentor and coach with others in the education process.

• Collaborates in a positive manner with physicians, nursing, other health care professionals and clerical staff as indicated regarding patient care and quality of care issues and communications, team building and change theory.

• Participates and provides in-service education dealing with aspects of care in area of this specialization with patient care staff within and outside of the Center.

• Is actively involved in organization wide activities, committees, task forces and special projects in an effective and positive manner to enhance the care of patients and to promote the care process as best practice.

• Participate in all community based active research and education.


Quality Improvement, Statistical & Financial Principle

• Manages resources/caseload to maintain productivity by maintaining medical record documentation time at a minimum.

• Provides accurate statistical and financial data as assigned and necessary. Demonstrates knowledge and skill in data collection, performance improvement and problem solving.


Self-Development & Other Duties Assigned

• Maintains current knowledge regarding management of patients with diabetes

• Assures all policies and procedures are current by participating in reviewing, revising and developing policies and procedures.

• Demonstrates flexibility in accepting schedule and work assignments.

• Keeps supervisor informed when problems may interfere with work being completed on time.

• Adapts to changes or unusual circumstances to promote cooperation and minimize disruption.

• During light workload situations, seeks out additional tasks/duties for the department.

• Performs other duties as assigned.


Clinical Care:

Ensure that quality patient care is delivered to all patients on a 24-hour basis, and the unit is maintained in accordance with professional and regulatory standards and the philosophy and goals of Hoag.


Education, Training and Experience


• 2+ years of clinical nursing experience

• 2+ years in diabetes management



Experience with various patient populations with diabetes

• BSN or equivalent

• 2+ years of experience as certified diabetes educator


Skills or Other Qualifications


Demonstrates effective communication, interpersonal skills, organizational ability, knowledge of proper body mechanics, safety measures and infection control, ability to follow instructions, knowledge of section specific test information and instrumentation may be required. Obtains higher learning experiences to enhance and develop new skills and knowledge to provide care through additional training and experiences. Cross trained for all aspects of office duties. Participates in special projects including but not limited to research and development, upon request. Adapts to changing needs in office to facilitate operational needs and improved access. Preferred: Demonstrates clinical expertise specific to age and disease specialty of population served. Excellent organizational, communication and interpersonal skills. Ability to work autonomously in a self-directed manner.


License and Certifications


• Current California license as a registered nurse in good standing

• BLS certification required

• Certified as Diabetes Educator (CDE)
