PATIENT CARE CLERICAL COORD - CHEMICAL DEPENDENCY

Under the direction of a licensed registered nurse, the Patient Care Assistant/Clerical Coordinator (PCA) assists in direct patient care by providing for activities of daily living (ADL), comfort, nutrition, and hygiene needs for the adult and geriatric patient. The Patient Care Assistant takes and records clinical measures, collects specimens, provides transportation, maintains equipment and supplies, and may assist in preparing charts. The PCA immediately informs the RN of any changes in patient condition and reinforces patient teaching as part of providing a safe patient-centered care environment.

 

Processes orders as directed by the RN for STAT, asap orders, verified orders, and provides follow-up as needed. Maintains and updates pertinent patient information in computer systems. Answers all phone calls and greets visitors. Manages call lights and patient needs.

 

Job Specific Essential Functions:

 

 

  • Process orders accurately and timely for tests, procedures, and special requests accurately during routine operations and downtime procedures.
  • Rounding on all patients and rooms every two hours and more often as needed to confirm patient status. Report change in patient behavior, vitals, mood or disposition to the Primary RN and Lead RN. Perform ability to recognize subtle changes in the neurobehavioral health patient’s mood or presentation and report changes to the RN team.
  • Ensures accurate and timely completion of forms to expedite patient care.
  • Maintains and updates pertinent patient information on computer systems, communication venues, and with appropriate departments in a timely manner.
  • Organizes and incorporates multiple tasks and skills with a high level of accuracy and efficiency.
  • Routinely checks and orders supplies, following-up to ensure stock is available to meet the needs of the unit and informing appropriate personnel if items are unattainable.
  • Wipe down all common areas every two hours and after all meals with appropriate cleaning solutions per infection prevention practices. Remove meal-trays from pt access at or less than 2 hours after delivery.
  • Directs ancillary staff that arrives on the unit for patient pick up. Greets All Patient Visitors.
  • Transport patients to the ED if applicable in appropriate equipment, wheelchair or gurney based upon pt status as directed by Lead RN. Use appropriate body mechanics to prevent injury.
  • Duties include monitoring patients that have a sitter order for safety, assist patients with an incentive spirometer, ambulation per policy, hygiene, safety and assist patients to a chair for all meals/eating.
  • Complete shift inventory of all pulse-ox equipment.
  • Tailors' communication style to meet the needs of the population served in a supportive, therapeutic manner.
  • Answers the telephone/call light promptly and courteously, relaying messages in a timely manner to the appropriate care giver.
  • Helps coordinate unit activities and groups for the unit and Counseling staff.
  • Notifies nurses in a timely manner of patients going off the unit, assures that appropriate forms are prepared and, in the chart, and obtains appropriate diagnostic data for patient records.
  • Ensures appropriate follow-up and resolution of issues and care. Provide hand-off report to oncoming shift and Lead RN.
  • Demonstrates the knowledge and skills necessary to provide care for assigned patients: accurately measures and records vital signs, I&O, height and weight, and ADL and records in the patient record, ensures personal hygiene of patient is maintained and documented.
  • Performs Accu-Check blood glucose test as required (must have Limited Phlebotomy license or RN license for this procedure).
  • Immediately informs RN of any changes in patient condition from the previous baseline.
  • Under the direction of the RN, performs routine patient admission and discharge duties and documents in the patient record per specific admission and discharge work-flows specific to the detox unit that includes a personal belongings search of all patient belongings. 
  • Assists in the reinforcement of patient teaching, rules of the unit and reports patient’s questions to the RN.
  • Perform other duties as assigned

 

Education, Training, and Experience

 

Required:

  • High School diploma or equivalent

 

Skills or Other Qualifications:

 

Required:

  • Demonstrates effective communication, ability to follow instructions, interpersonal skills, and knowledge of proper body mechanics, safety measures, and infection control. Must be conversant in English and be able to accurately document in the patient electronic medical record (computerized chart).

 

Preferred:

  • Knowledge of section-specific test information and instrumentation.

 

License and Certifications:

 

Required:

  • Current certification as a Nursing Assistant/CNA from the State of California, or completion of the first semester of Registered Nurse program or equivalent.
  • Current BLS certification is required prior to clinical rotation.
  • Must obtain Limited Phlebotomy certification within one year of hire as a PCA.
