Job Description:

The Nurse Practitioner works collaboratively with the physicians in providing multidisciplinary care for adult and geriatric patients requiring services in both the inpatient and outpatient setting of the hospital. The Nurse Practitioner, in collaboration with the medical staff, delivers supportive medical management for a defined patient population according to Hoag standards and regulatory/governing requirements. In this role, the Nurse Practitioner maintains a system for coordinating caseload of care throughout the entire episode of hospitalization and illness, spanning each clinical area in which care is provided and manages each patient’s transition through the system. Provides primary care within the scope of the Nurse Practitioner license with emphasis on health promotion, education, coordination of care and emotional support to the patients and families while combining the qualities of both nursing and medicine. Acts as a patient advocate in all ethical and legal aspects. In addition, the Nurse Practitioner plans, coordinates and provides for optimum patient care in the clinic. This includes participating in the facilitation and authorization process for chemotherapy, immunotherapy infusions, and clinical trials; office schedules and patient assignments; coaching and mentoring team members; chart reviews to determine best practice and outcomes. Serves as a liaison for decision making on behalf of the physician in their absence. Ensures physician’s mandated hospital and training requirements are met. The Nurse Practitioner attends and is an active member of hospital committees and meetings affecting the clinic patient population and outcomes.

 

Clinical Practice:

Job Responsibilities: (*denotes privileges granted by the Medical Staff Department of Hoag Hospital)

 Performs history and physicals*

 Furnishes medications (non-narcotic)*

 Orders laboratory tests*

 Dictates/documents follow-up consult notes*

 Orders necessary imaging studies, home health, physical therapy*

 Documents in the patient record utilizing documentation guidelines, standards of care and patient goals.

 Performs follow-up at specific intervals per specific algorithms and protocols

 

Essential Functions:

 Works in collaboration with the medical and Hoag staff to provide multidisciplinary care of patients.

 Provides clinical, professional expertise and leadership for patients, families and staff members.

 Functions as an expert clinician, consultant, educator and leader in brain tumor patient care management, ensuring optimal patient care management and outcomes.

 Maintains a system for coordinating the caseload of care throughout the entire continuum of care spanning each clinical area where care is provided and manages each patient’s transition through the system.

 Acts as a consultant in clinical setting, maintaining and promoting standards of care.

 Facilitates access to services and timely movement through points of care.

 Phone follow-up at specified intervals and at points of transition in care.

 Participates in the facilitation and authorization process for chemotherapy, immunotherapy infusions, and clinical trials

 Maintains active involvement in nursing and hospital committees as necessary.

 

Clinical Care:

• Ensure patient care is delivered and the unit is maintained in accordance with professional regulatory standards. Demonstrate the knowledge and skills necessary to provide care appropriate population as outlined in the job description and competencies

 Performs other duties as assigned

 

Education, Training and Experience

Required:

 Master’s degree in Nursing.

 Graduate of a Nurse Practitioner Program

 Current license in good standing to practice as a Registered Nurse/Nurse Practitioner in California along with Continuing Education Requirements.

 Adult Neurology/Oncology working as a Nurse/Nurse Practitioner or equivalent combination of relevant education and/or experience

 Furnishing license issued by the State of California and personal DEA number.

 Current ACLS and BLS.

 

Preferred:

 ONS Certification

 

Skills or Other Qualifications

Required:

 Evidence of satisfactory performance and completion of proctoring requirements within 6 months of hire.

 Availability and appropriate use of consulting physician on medical staff for Hoag.

 Performs according to approved standardized procedures and/or protocols.

 Assessment of patient’s status with appropriate findings and initiation of appropriate tests, medications and therapeutic interventions.

 Excellent communication skills; customer focused; excellent attention to detail; able to prioritize tasks and meet deadlines; strong interpersonal skills; able to work in a team environment.

 Proven excellent customer service skills, including the ability to progressively probe, analyze, identify root causes of problems, provide practical solutions, and negotiate resolutions.

 Solid computer technical skills required, including proficiency with Microsoft Office products (Outlook, Word, Excel, PowerPoint, and Visio).

 

Preferred:

 Qualified bilingual or multilingual candidates.

 

License and Certifications

Required:

 Current California Registered Nurse/Nurse Practitioner license in good standing.

 Current BLS.
