MANAGER CLINICAL SERVICES - HOME MD - HAH PHYSICIANS

Primary Duties and Responsibilities

Hoag Clinic:

The Hoag at Home-Manager of Clinical Services functions as a proactive leader, educator, resource, consultant, and researcher with regard to clinical aspects of Hoag Clinic business, in close collaboration with and under the direction of the Director of Clinical Services and other members of the Hoag Clinic leadership team. The Manager of Clinical Services works to ensure optimal patient care outcomes and drive innovation. The scope of the Manager of Clinical Services’ responsibility includes but is not limited to: evidence-based patient care standards; staff education; patient and staff safety; regulatory compliance; risk management; patient relations; and clinical workflow design.

 

Home Health/Hospice/Home MD:

The Manager of Clinical Services functions as a proactive leader, educator, resource, consultant, with regard to clinical aspects of the Hoag at Home MD  business, in close collaboration with and under the direction of the Director of Post-Acute Care and other members of the Hoag at Home leadership team. The Manager of Clinical Services works to ensure efficient office practices, effective routing of clinical staff, scheduling, operational management of the medical assistants and other care providers, and ensures optimal patient care outcomes and drives post-acute innovation. The scope of the Manager of Clinical Services’ responsibility includes but is not limited to: evidence-based patient care standards; staff education; patient and staff safety; regulatory compliance; risk management; patient relations; and clinical workflow design.

Education and Experience

Hoag Clinic: Bachelor of Science in Nursing (BSN). Four (4) years clinical nursing experience; Two (2) years in an ambulatory care setting; Two (2) years’ experience in leadership, staff education, or risk management. Intermediate knowledge of Microsoft Office Suite, including Outlook, Word and Excel. Strong clinical documentation knowledge and skills

Preferred:

 

Master of Science in Nursing (MSN)

Home Health/Hospice/Home MD: Five (5) years’ experience in Home Health, Hospice, Palliative Care, or MD office operations.

 

Licenses Required

Hoag Clinic: Current licensure as a Registered Nurse in good standing in the State of California

Home Health/Hospice/Home MD: Current licensure as a Registered Nurse in good standing in the State of California OR LCSW - Licensed Social Worker (LCSW), in good standing, in the State of California. Master’s Degree in Social Work (MSW) will be considered in lieu of RN or LCSW.
