ounselor II is part of the Hoag Addiction Treatment Center (HATC) team supporting the assessment, treatment planning, referral recommendations, discharge planning, education, and direct therapeutic for patients and their families throughout the course of treatment. This role supports the goals of the patient's treatment plan and assists in clinical care and case management. Coordinates care with outside providers by way of referrals when necessary. 

Serves as a point of contact for the patient between Therapists and other members of the treatment team. 

Collaborates with the Medical Director, Clinical Supervisor, Supervisor of Outpatient Services, Operations Manager, and all members of the Hoag Addiction Treatment Center multi-disciplinary team.

Participates in on-going process improvement and patient satisfaction initiatives. 


Essential Functions:

  • Leads and facilitates lectures for clients. 
  • Ensure the clients follow the treatment schedule. 
  • Assist with admissions and discharges, assuring all admissions and discharge documents are signed. 
  • Provides clients with an orientation about the program, rules and, a tour of the unit, including their room assigned. 
  • Accompanies clients to and from medical and other appointments or other critical needs as they arise. 
  • Redirecting and reporting clients if rules are broken. 
  • Responsible for understanding and enforcing program rules at all levels of care. 
  • Assists in searching belongings at the time of admission as well as random room searches in the residential level of care and collect USD as assigned. 
  • Responsible for reading the shift log at beginning of a shift and documenting all necessary information at the end of the shift. 
  • Receives and presents a change of shift reports as assigned. 
  • Communicates with physician and supervisor of outpatient services with regards to physician orders, prescriptions, and refills as needed. 
  • Observe self-administration of medications and document in client EMR. 
  • Document daily living activities and vital information.
  • Records patient behaviors and inform Supervisor of Outpatient Services and Clinical Supervisor of any changes in client's behavior. 
  • Assists and encourages clients to participate in self-help groups, such as AA/NA, which offer clients an opportunity to support one another in recovery.
  • Types and performs data entry as needed for communication purposes. 
  • Performs other duties as assigned. 


Education, Training, and Experience:



  • High School Graduate or equivalent required. 
  • Minimum of one-year work experience in an alcohol/drug dependency treatment program. 
  • Working knowledge of CARF and accreditation standards. 
  • Knowledge of the 12-Step, Smart Recovery, and other treatment models.
  • Knowledge of HIPPA.



  • Experience in Addiction/Behavioral Health environment. 





  • BLS through American Heart Association
