Counselor II, Per Diem - Solmar

The Counselor II supports the Hoag Addiction Treatment Center (HATC) treatment team in the assessment, treatment planning, referral recommendations, discharge planning, education and direct therapeutic intervention for patient and their families throughout the course of treatment. This role supports the goals of the patient’s treatment plan and assists in the clinical care and case management. Coordinates care with outside providers by way of referrals when necessary. Serves as a point of contact for the patient between therapists and other members of the treatment team. Collaborates with the Medical Director, Clinical Supervisor, Supervisor of Outpatient Services, Operations Manager and all members of the HATC multidisciplinary team. Participates in on-going process improvement and patient satisfaction initiatives.

 

Essential Functions:

  • Conducts group counseling sessions and manages group sessions as assigned. Leads and facilitates lectures for clients.
  • Ensure all documentation regarding patient care, group counseling sessions treatment and incidents are completed in a timely and in a clear concise manner.
  • Assist with admissions and discharges, assuring all admissions and discharge documents are signed.  Provides client with an orientation about the program, rules and a tour of the unit, including: their room as assigned.
  • Assist on searching belongings at the time of admission as well as random room searches in residential level of care.
  • Intervene as an advocate for clients or patients to resolve emergency problems in crisis situations. Intervene in crisis situations to assist clients.
  • Collaborate with therapist and treatment team to assess client needs and treatment plans. Act as a liaison between medical staff, therapists and the rest of the treatment team.
  • Communicating with physician and supervisor of outpatient services in regard to physician orders, prescriptions and refills as needed. Observe self-administration of medications and document in client EMR.
  • Document daily living activities and vital information. Record patient behaviors and inform Supervisor of Outpatient Services and Clinical Supervisor of any changes in behavior.
  • Responsible for reading the shift log at beginning of shift and documenting all necessary information at the end of the shift. Receives and presents change of shift reports as assigned.
  • Attend training sessions to increase knowledge and skills. Participate in case conferences or staff meetings.
  • Lead and coordinate the community meeting for clients (residential level of care). Types and performs data entry as needed for communication purposes
  • Assist and encourage clients to participate in self-help groups, such as AA/NA, which offer clients an opportunity to support one another in recovery.
  • Assist and document times when clients are getting their cell phones and assure client’s cell phones are collected after the allocated time is over.
  • Travel – Position may require local travel between various Organization locations.
  • Perform other duties as assigned.

 

 

Job Requirements:

  • Associate degree in Behavioral Science or Allied Mental Health Profession.
  • Certification as Addiction Certified Counselor I, or II (CADC-II) or Certified Addiction Treatment Counselor II (CATC-II) required.
  • At least 2 years of experience in addiction/behavioral health environment preferred.
  • Experience in addiction/behavioral health environment preferred.
  • Working knowledge of CARF and/or,  DNV and/or JACHO accreditation standards.
  • Knowledge of the 12-step, Smart Recovery and other treatment models
  • Knowledge of HIPPA
  • Demonstrates excellent, effective communication, interpersonal skills, organizational ability, and ability to follow instructions.
  • High degree of organizational skills. Able to set assignment priorities, work independently within established guidelines and maintain confidentiality
  • Creative problem-solving skills and abilities.
  • Ability to develop and maintain collaborative and synergistic relationships across multiple customers.
