CLINICAL NURSE NAVIGATOR I - BREAST CARE & IMAGING CENTER

Salary Range: $55.2400 - $84.9400 /hour. Actual compensation may vary based on geographic location, work experience, skill level, and education.

 

The Clinical Nurse Navigator I is a registered nurse responsible for the coordination of care across the continuum for patients with complex, integration sensitive diagnoses. The navigator may coordinate care from time of diagnosis through end of life. Educates patient and family/significant other on the hospital experience including preadmission and post discharge planning; implements, coordinates, monitors and evaluates care options, services and resources. Links patient/family to appropriate resources as needed.

 

 Essential Functions

• Proactively anticipates patient care needs.

• Improves patient access to care by navigating through identified barriers.

• Enhances patient experience through coordination of care across the continuum (MD office to acute experience to chronic care management and survivorship follow up).

• Links patient to appropriate settings or services to promote speed and efficacy of care, e.g., improve timeliness of diagnostic tests.

• Assesses psychosocial needs, including emotional and situational support of the patient/family, and appropriately coordinates resources to manage.

• Actively participates in consensus planning with interdisciplinary care management team to assist in formulating the plan of care and providing information for an informed decision.

• Under direction of the physician/team, uses evidence-based best practices in the care and management of patients.

• Under direction of the physician/health care provider (HCP), implements chronic care management care plan to assure optimal quality of life outcomes.

Patient:

• Assists patient/family in understanding their diagnosis, treatment options, and resources available.

• Assist with the designs and of educational programs and tools for patients, families and groups.

• In collaboration with HCP, improves prevention and early detection efforts by participating in community education forums and screenings.

• Assist with the education of staff members utilizing standardized protocols in consensus with national guidelines.

• Collaborates with the multidisciplinary team to provide education to family/caregivers to enhance the patient experience and to promote optimal outcomes.

Quality:

• Assist the health care team in quality metrics, by gathering data.

• Assist along the health care team in tracking data, outcomes and supporting strategic and performance improvement activities.

• Actively participates in clinical trials by assisting the clinical research team to identify patients who meet clinical trials criteria.

Community & Physician Outreach:

• Manages transitions in patient care through effective coordination and communication with HCP and their office staff.

• Leads support groups and monitors outcomes, e.g., patient satisfaction, patient compliance with treatment regimen and loyalty.

• Educates referring HCP and facilities on available services

• Performs other duties as assigned.

 

Job Requirements:

  • Current Registered nursing license to practice in the state of California. Minimum of 5 years in nursing. Minimum of 3 years in area of specialty.
  • Current California license as a registered nurse, in good standing. Current BLS prior to employment.
  • Demonstrates effective communication, interpersonal skills, organizational ability, time management, knowledge of proper body mechanics, safety measures and infection control, ability to follow instructions, complies with regulatory guidelines and rules, positive, can-do, pro-active attitude. Current experience in the cancer arena or hospital based care. Familiar with community resources and hospital processes, structure and function. Strong knowledge of body systems and physiology. Detail oriented, strong documentation skills, clinically astute. Comfortable with technology and innovation.
  • In addition to the above functions the clinical nurse navigator II performs these additional functions

 

Job Preferences:

  • Must have critical thinking skills, be able to problem solve independently, work well in a team environment Strong written and verbal communication skills.
  • Competent in Microsoft Office, keyboard a minimum of 45 WPM.
  • Current use or recent past use of an electronic charting method/EMR.
  • Prior experience as a case manager, navigator or heath educator.
