tilizes the nursing process based on research and evidence-based outcomes to meet patient care needs. Participates in maintaining a safe patient care environment.  Demonstrates leadership skills by directing personnel and establishing collegial relationships with others. Provides quality nursing care to all patients in accordance with the California Nurse Practice Act, and professional regulatory standards. Displays and practices a work-style reflecting the mission, vision, and values of the department and the Organization. Participates in the continuous quality improvement process. Maintains a working knowledge of departmental standard operating procedures, including the use of specialized instrumentation, Quality Control requirements, and preventive maintenance.

The patient population served is the adolescent, adult, and geriatric patient with rehabilitation diagnoses and associated deficits.  The primary responsibility of the nursing staff is to provide direct patient care and to collaborate with other disciplines of the health care team to ensure the delivery of safe, timely, and appropriate quality patient care.  In addition, the registered nurse fosters self-management to assist the patient and family with post-acute care transitions to promote and sustain well-being through risk reduction, harm prevention, and maintenance of optimal health.


Clinical Nurse I (RN)

  • New hires, whether experienced or new graduate nurses, will correspond to the "novice" nurse in Benner's model for the initial six months. 
  • During the first 6 months of employment, the new hire will obtain knowledge and experience in clinical and technical skills. Under the guidance of a preceptor, charge nurse, nurse manager, and/or educator, the new hire collects objective data and seeks assistance in making clinical decisions.
  • After successfully completing the 6-month probationary period the Clinical Nurse I will correspond to the "advanced beginner" in Benner’s model. 
  • Guided by policies, procedures, and standards. The Clinical Nurse I (RN) is building a knowledge base through practice and are most comfortable in a task environment. 
  • Describes a clinical situation from the viewpoint of what they need to do rather than relating the context of the situation or how the patient responds. 
  • Practices from a theoretical knowledge base while they recognize and provide for routine patient needs.



Essential Job Functions:

Clinical Practice

  • Demonstrates competent knowledge, based on Organization and policies, procedures, and standards.
  • Demonstrates an ability to organize and incorporate new routines, procedures, and skills.
  • Demonstrates skill in the use of equipment and devices pertinent to treatment, procedures, and practices.
  • Demonstrates the use of the nursing process as documented on the admission assessment, patient Plan of Care, focus charting, and discharge plan.
  • Assesses and documents a complete physical assessment, nursing interventions, teaching and discharge needs for the patients assigned to each shift.
  • Documents in the patient record utilizing documentation guidelines, Standards of Care, and Pathway goals.
  • Communicates changes in the patient's condition to the Charge Nurse/supervisor and the physician directing the care of the patient as well as pertinent information at shift report using the patient Plan of Care.
  • Follows procedure in the identification of patients prior to implementing any procedures, interventions, etc.
  • Provides appropriate information for all patient procedures and ensures patient/significant other understanding prior to the start of the procedure. 
  • Responds to emergency situations in a timeframe appropriate to the needs of the patient.
  • Administers medications and intravenous therapy with the ability to assess the indication of appropriate dosing and effects of the medication administered for the patient.
  • Initiates IV therapy and documents accurate information on the patient record.
  • Correctly collects, handles, and labels specimens utilizing Standard Precautions, as well as documenting in the patient record.
  • Adheres to Infection Control policies and procedures, follows Standard Precautions and specific isolation precautions for the prevention and spread of blood-borne pathogens and other communicable diseases.
  • Appropriately assesses and implements prescribed interventions to ensure pain management for the patient.
  • Recognizes the ethical/legal/medical dilemmas existing in the nursing practice and confers with the appropriate resource personnel when making ethical nursing decisions. 
  • Implements and documents teaching discharge plan for each patient.
  • Assesses the educational ability and level of the patient population.  Incorporates available educational material into patient/significant other teaching and documents understanding in the patient record.
  • Provides ongoing assessment of the nutritional status of the patient.  Implements dietary consult when indicated to provide/maintain adequate nutritional intake.  Communicates to physician nutritional needs/problems for the patient.
  • Provides continuity of care through implementation/utilization of the Plan of Care for the patients.
  • Appropriately assesses, intervenes, and cares for the adult/geriatric patient, family, and significant other requiring emotional support, coping support, and care for significant life changes and end of life issues.  Coordinates support services with Social Services, Pastoral Care, Case Management, etc.
  • Clinical Care:  Ensure patient care is delivered and the unit is maintained in accordance with professional regulatory standards. Demonstrate the knowledge and skills necessary to provide care appropriate population as outlined in the job description and competencies.


Unit/Department Specific

Demonstrates competent knowledge, based on department policies, procedures, and standards, in the following: 

  • Demonstrates competency in Functional Improvement Measure (FIM) scoring.
  • Collaborates with the patient, family, and Interdisciplinary Team (IDT) to develop a Plan of Care (POC) with realistic goals.
  • Demonstrates understanding and carry through on therapists’ plan of care for the patient to include transfers, mobility, self-care, and swallowing techniques.
  • Collaborates with patients and caregivers to facilitate post-acute care transitions through the promotion of optimal health, self-management, quality of life, risk reduction, and harm prevention.
  • Demonstrates critical thinking in the identification of clinical, social, safety, psychological and spiritual issues for the patient care within their rehab admission.
  • Performs other duties as assigned.


Education, Training, and Experience


Required: Associate’s Degree in Nursing and must complete a Bachelor’s of Science in Nursing from an accredited school of nursing within two years of the hire date.


Preferred:  Bachelors of Science in Nursing from an accredited school of nursing.  Previous experience in rehabilitation nursing. Certified Rehabilitation Registered Nurse.


Skills or Other Qualifications


Required: Demonstrates effective communication, interpersonal skills, organizational ability, knowledge of proper body mechanics, safety measures and infection control, ability to follow instructions, knowledge of the age of population served.  Ability to collaborate, communicate, and partner with patients and family members to the extent they need and desire.


  • Qualified bilingual or multilingual candidates.
  • Working knowledge of Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint, plus healthcare database systems.



License and Certifications



  • Current licensure, or interim permit as a Registered Nurse in the State of California required.
  • Current BLS certification is required prior to clinical rotation


  • FIM certification.
  • Certification in rehab nursing (CRRN). NIHSS certification
