The Triage Nurse (RN) I/II provides professional telephone triage nursing services to Hoag Clinic patients. Utilizes the nursing process based on research and evidence-based outcomes to meet patient care needs. Collaborates with other members of the health care team to ensure the delivery of safe, timely, appropriate, quality patient care. Facilitates appropriate utilization of healthcare resources. Enhances the patient experience. Provides services in compliance with all applicable regulatory and professional standards, including the California Nurse Practice Act.


Triage Nurse (RN) I:

• Performs telephone triage. Performs detailed evaluation of patient symptoms and determines urgency of patient condition. Facilitates appropriate next steps in patient care, including scheduling visit with health care provider, making recommendations for escalation of care, and/or providing appropriate medical advice on treating symptoms. Utilizes approved clinical decision making tools and algorithms. Consults with health care provider appropriately.

• Plans, implements, and evaluates patient education. Assesses health literacy of learner(s) and modifies education appropriately. Incorporates available education material.

• Communicates changes in patient condition and other pertinent information to the health care provider directing the care of the patient.

• Responds to emergency situations in an appropriate timeframe.

• Documents all nursing services provided in the patient record.

• Coordinates or facilitates support services with Clinical Nurse Navigator and/or Licensed Clinical Social Worker as appropriate.

• Confers with appropriate resource personnel when facing ethical or legal issues.

• Serves as a clinical resource and professional role model for other staff.

• Participates in the continuous quality improvement process.

• Performs other duties as assigned.

Triage Nurse (RN) II:

• Educates and guides other staff to solve complex patient care problems.

• Conducts and/or coordinates department orientation and training of new employees.

• Develops policies, procedures, standardized procedures, and protocols.

• Leads and coordinates innovate changes and improvements.


Education, Training and Experience

Triage Nurse (RN) I: Required: Two (2) years clinical nursing experience.

Preferred: One (1) year experience in an ambulatory care setting and/or in telephone triage. Bachelors of Science in Nursing (BSN).

Triage Nurse (RN) II: Required: Seven (7) years clinical nursing experience. BSN.

Preferred: Two (2) years experience in an ambulatory care setting and/or in telephone triage.


Skills or Other Qualifications


• Basic keyboarding skills.

• Basic knowledge of Microsoft Office Suite, including Outlook, Word and Excel.

• Strong clinical documentation knowledge and skills.

• Proven ability to interface with all customer levels (physician, organization staff, insurance companies, patient and family members).

• Strong critical thinking, trouble-shooting, and problem-solving skills.

• Effective communication skills, interpersonal skills, and organizational ability.

• Knowledge of proper body mechanics, safety measures and infection control.

• Knowledge of age of population served.

• Additional skills and knowledge of equipment may be required for hire into some patient care departments.


• Knowledge of electronic medical record systems.

• Knowledge of patient registration, scheduling, and insurance verification processes.

• Intermediate knowledge of Microsoft Office Suite, including Outlook, Word and Excel.


License and Certifications

Required: Current licensure as a Registered Nurse in good standing in the State of California. Current BLS certification.
